Application Pre-Screening

I am 18 years of age or older.
I am a legal resident of the United States.
I am here today to apply for help either for myself or my household, and not for someone else.
My household has at least one active bank account.
My Household Filed a Federal Tax Return in 2019.
I have regular access to a computer and the Internet and can check my email and the Needs & Wishes website daily for the entire time that my application is active.
I have the ability access either to a scanner or a fax machine at my home, my place of business, or a shop like FedEx Business or Office Max for long enough to send the documentation that Needs & Wishes will request from me during the grant application process to Needs & Wishes' office for evaluation.

If you rent (rather than own) your residence:

My household has a legal lease that my landlord and a member of my household both have signed. This lease shows the monthly cost of my apartment, whether or not I pay all of that amount.
